FINANCIAL BLUEPRINT REVIEW AGREEMENT
Christy Capital Management, Inc. (“CCM”) and the Client whose name(s) and address are set forth below agree that CCM will provide to Client a Financial Review in exchange for a fee in accordance with the following terms.
Process. Client agrees to provide the personal and financial information requested in the CCM
Financial Review Intake Form and to pay CCM $497.00 upon submission of that form to CCM. Client agrees to promptly respond to any questions or requests for information received from a CCM Representative. CCM shall, within 15 business days after the latter of the receipt of the Intake Form or the provision of additional information to CCM, schedule a Consultation to deliver and discuss the Financial Review with a CCM Financial Planner.

Product. The Financial Report shall include a Risk Tolerance Analysis, a Financial Goals Review, Current Investment Allocations, and a resulting Asset Allocation Blueprint. The Financial Review is limited in scope, is not intended to be a comprehensive financial plan, and will not be updated or periodically reviewed by CCM after its delivery. Client may separately engage CCM for investment advisory services, including asset management, and/or for a comprehensive financial plan.

Privacy & Disclosures. CCM will treat Client’s information as confidential and will not disclose it to any other person, except as authorized by Client or required by law. CCM is an SEC-registered investment advisor and is required to provide certain disclosures to Client. These are the Form CRS (customer relationship summary), Form ADV Parts 2A and 2B, and our privacy notice. By signing this Agreement, Client acknowledges receipt of these disclosure documents and the opportunity to review and ask questions about each.

Reliance. Client acknowledges that the CCM analysis and recommendations are dependent upon the accuracy and completeness of the information Client provides. Client agrees that CCM is entitled to rely upon the information supplied by Client in preparing the Financial Review.

Additional Terms and Conditions., Client and Christy Capital Management agree that this Agreement and the additional terms and conditions located HERE govern their relationship.
